Transition Coordinator (backfill)
Job Responsibilities may include, but are not limited to:Facilitate the implementation of a life skills curriculum, titled Next Steps.Utilize the Transition Coordinator Handbook to standardize transition practices.Evaluate the five "Transition Consultations" for each student under teacher guidance.Participate weekly in Administrative Release Committees (ARC) to reinforce students' reentry plans.Conduct follow-up phone calls with released youth at 1, 3, 6, 9, and 12 months.Represent the Indiana Department of Correction- Division of Youth Services at conferences and meetings.Assist in developing partnerships with Indiana's Department of Workforce Development, HIRE, and Indiana University's HOPE Mentoring.Assist in developing individualized Education Learning Plans for each youth upon entry.Collaborate with state, county, and private agencies to provide re-entry services for transitioning youth.Assist students with obtaining necessary documentation such as social security cards and state identification.Support students in transitioning back to public schools.Provide information on employment, apprenticeship programs, and military service.Skills and ExperienceAbility to communicate effectively in a team environment. RequiredExperience with training, coaching, or guiding youth. RequiredExperience collaborating and creating initiatives. RequiredExperience as a spokesperson at events.
