Registered Nurse - Wound Care

The Wound/Ostomy RN functions as a clinical consultant for wound, skin, and ostomy management across all inpatient areas, including med/surg acute care units, ICU, long-term care, psychiatric, and step-down units. The RN provides comprehensive assessment, treatment planning, patient education, and coordination of care for Veterans with acute and chronic wound and ostomy needs.Respond to wound/ostomy consults and provide specialized assessment and treatment.Recommend referrals to interdisciplinary services as clinically indicated.Provide age-appropriate patient, family, and caregiver education.Initiate wound therapies per NF/SG Wound/Ostomy RN Scope of Practice and Ordering Protocol.Perform conservative sharp debridement following additional education and competency review.Apply advanced wound therapies including negative pressure therapy and specialty dressings.Coordinate supply ordering, home health needs, and continuity of care.Assess and recommend specialty support surfaces for Veterans at risk of skin breakdown.Provide preoperative ostomy education, stoma site marking, and postoperative instruction.Pressure injury identification as part of OIG audit mandate, NPIAP recommendations, and SAIL/local incidence monitoring.
