Nurse Practitioner - Primary Care Position Details Position Type: Full Time Shift: Day Education Required: Graduate Degree Travel: Up to 25% Position Overview Seeking a full-time Family Nurse Practitioner (FNP) to provide comprehensive primary care services for adult and pediatric populations. Responsibilities include diagnosis, treatment, preventive care, chronic disease management, care coordination, and health maintenance. The provider will work collaboratively within a multidisciplinary medical team while maintaining compliance with all applicable policies, procedures, and state/federal regulations.Responsibilities Clinical Care Provide comprehensive primary care across the lifespan, including health assessments, physical exams, diagnostic evaluations, and management of acute and chronic conditions using evidence-based guidelines. Order, perform, and interpret diagnostic tests such as blood work, imaging, EKGs, and urinalysis. Develop and adjust individualized treatment plans, including medications, therapies, and preventive interventions. Deliver preventive services including wellness visits, vaccinations, screenings, and routine check-ups. Educate patients on disease prevention, healthy lifestyle habits, and self-care strategies. Prepare and administer medications, injections, and vaccinations. Review lab results with patients and treatment teams. Evaluate behavioral health assessments and coordinate referrals when needed. Perform office-based procedures within scope of practice and participate in urgent/acute care situations. Care Integration & Collaboration Collaborate with physicians, nurses, behavioral health clinicians, care coordinators, pharmacists, and ancillary staff to ensure continuity of care. Participate in daily huddles, case conferences, and care planning meetings. Coordinate referrals and transitions of care across healthcare settings. Quality & Compliance Maintain accurate and timely electronic health records and clinical documentation. Support quality metrics, compliant billing, and medical necessity documentation. Participate in quality improvement initiatives and population health management. Meet productivity, patient access, and panel management expectations. Adhere to confidentiality standards, medical ethics, and state regulations related to collaborative practice and prescriptive authority. Professional Development Stay current with medical research, clinical guidelines, and evidence-based practices through continuing education. Support the education and development of healthcare team members. Qualifications Required Qualifications Licensed Advanced Practice Nurse in Colorado or compact state. Current DEA license. Current CPR certification; ACLS or Advanced Life Support certification as required. Minimum 5 years of RN experience and 3 years of Family Nurse Practitioner experience. Eligible for malpractice coverage. Computer proficiency. Strong patient-centered communication and education skills. Demonstrated professionalism, compassion, flexibility, and teamwork. Preferred Qualifications Experience caring for patients with addictions and severe/persistent mental illness. Experience working within multidisciplinary teams. Managed care experience. Spanish language skills. Physical Requirements Ability to stand and walk for extended periods. Ability to lift or assist patients and move equipment as needed. Frequent bending, stooping, twisting, reaching, and positioning during patient care. Fine motor skills for procedures including injections, suturing, and equipment operation. Visual and auditory acuity for patient assessment and communication. Clear communication and accurate documentation skills.
