Position OverviewThe FNP serves as a core member of the Medical Services Team, providing a full scope of primary care services across adult and pediatric populations. This position operates under an integrated behavioral health model, delivering patient-centered care aligned with Patient-Centered Medical Home (PCMH) principles and Alternative Payment Model (APM) metrics. Key ResponsibilitiesComprehensive Care: Deliver full-scope primary care across the lifespan, including complete health assessments, physical examinations, diagnostic evaluations, and management of acute/chronic illnesses. Diagnostics & Treatment: Order, perform, and interpret diagnostic tests (e.g., blood work, imaging, EKGs) and prescribe pharmacological therapies using evidence-based guidelines.Integrated Care Collaboration: Partner closely with internal psychiatric nurse practitioners, behavioral health clinicians, and case managers to address complex co-occurring medical and mental health conditions.Documentation: Facilely utilize electronic health records (EHR) and electronic prescribing systems (NextGen/MyAvatar) to accurately log care and ensure billing alignment. Role RequirementsEducation: Graduate Degree from an accredited Family Nurse Practitioner program. Licensure: Valid Advanced Practice Registered Nurse (APRN) and FNP licensure in the state of Colorado; prescriptive authority preferred.Certifications: Current CPR / BLS Certification.
