This position involves coordinating patient care, conducting in-home visits, and managing care plans across a diverse patient population.Candidates must have at least 2 years of nursing experience, including recent Home Health experience, along with a background in care management. Proficiency in HCHB (Homecare Homebase) and OASIS documentation is required.
