Job DescriptionHoag Health, the top-ranked health system in Orange County, CA is seeking a Primary Care (Precision Medicine) Physician for its digital platform Hoag Compass. Hoag Compas is an innovative program built to provide best in class whole person care centered around longevity, preventative, and wellness-based care rooted in Functional Medicine principles. Compass pairs digital, virtual, and in-person experiences to provide patients the right level of care and attention when they need it most. The Hoag Compass Physician will play a key role in integrating best-in-class digital healthcare products for a thriving health organization in their patient care model.Position Details: We are a high-volume practice seeking an energetic, self-starter who can work independently and contribute to fast-based team in a five-star service environment.Full-Time opportunity in San Clemente, CA,Schedule: Monday Friday, 8:00am 5:00pmResponsible for managing a limited patient panel including individualized prevention, treatment planning, acute care, chronic care, coordination of care and overall wellnessConduct visits in-office and virtually via telehealth modalities as well as communicate with patients via secure communication platformsPrescribing or administering medication, therapy, and other specialized medical care to treat or prevent illness, disease, or injury.Collaborate with the digital team to ensure that all digital experiences and products are safe, effective, optimized for physicians in mind and are patient-centered.Partner with multidisciplinary care team of nutritionists, exercise physiologists, health coaches to build personalized wellness programs for patients.Work closely with clinical teams and stakeholders to identify areas where digital solutions or existing experiences can improve patient and provider experiences & outcomes.Partner with business leads to effectively communicate product vision to clinical teamEnsure that digital products comply with all relevant regulations and standards including HIPAA and other data privacy requirements.Stay current on the latest trend and technologies in digital healthcare and provide thought leaders on how to apply them in a hospital setting.Hoag Compass providers also demonstrate:A passion for improving care and the patient experienceA desire to stay current on the latest trends and technologies in digital healthcareAn openness to working collaboratively in ambiguous environmentsA willingness to adapt to changeQualifications Required:M.D. or D.O.
