Henry Ford Health is seeking experienced Home Health Registered Nurses to join the Community Care Services team. This role involves providing direct nursing care to patients in their homes, utilizing nursing principles to assess patient needs and integrate allied health services (PT, OT, Speech, Social Work) into a comprehensive care plan. This position is a "Peak in Census" backfill with a high volume of openings. Key Responsibilities Direct Patient Care: Provide high-quality nursing care within the home setting, managing a caseload of approximately 6 patient visits per day . Comprehensive Assessment: Evaluate patients for necessary allied health services to ensure an integrated delivery system. Travel: Anticipate driving an average of 40–60 miles per day across Wayne, Oakland, and Macomb counties. Coordination: Communicate effectively with patients, families, and interdisciplinary hospital personnel to ensure continuity of care. Schedule & Training Hours: 40 hours per week (8:00 AM – 4:30 PM). Weekend Rotation: One Saturday every month required. Orientation: 2–4 weeks (depending on acuity); held at 1 Ford Place and in the field with a preceptor.
