Job Description
Provides supportive and educational services to youth and adults with Intellectual/Developmental Disabilities who have the Home and Community Based waiver and/or Medicaid, including Medicaid MCOs. The Community Support Worker will provide support to individuals in their homes and the community by teaching skills that foster independence and social connections and promote mental health. Services are primarily offered in the northern part of Chicago and the northern suburbs.Knowledge, Skills and Responsibilities: Facilitate the individual’s participation in activities, skill development, or other supports identified in the Personal Plan, service plan or IMCANSProviding support can include, but is not limited to support around daily care, navigating the community/learning transportation; facilitating social and leisure opportunities, promotes interests and skill building; assists with identifying and accessing resources; and activities promote mental health.Support the well-being and safety of the individual.Advocate on behalf of and promote the individual’s ability to self-advocate.Be a part of a collaborative support system that may include family, friends, professionals, and community members.Regularly meet with supervisor and clinical team as appropriate.Complete accurate documentation of activities on a regular basis (daily, weekly).Provide and assist with transportation during meetings with individual.Attend team meetings as scheduled.Opportunities to be a part of other program activities/events.Other duties as assignedRequirements:Must be at least 21 years old, with a high school diploma or GED.Demonstrated ability to accept supervision and work constructively with individuals, colleagues and the communityDirect Support Professional (DSP) certified preferred, or ability to complete training to become certified within 60 days of hire. Ability to work a flexible yet consistent schedule (evenings and weekends required) Complete CPR/First Aid training. Complete Physical and Background Check. Must have access to a dependable car, valid Illinois Driver’s License, and auto insurance.
