Job Title: Social Work, Care Manager - LMSW/LCSW - Remote Are you a compassionate and experienced social worker who thrives in a collaborative care environment? We are seeking a dedicated Care Manager – Social Work to support a vulnerable and aging population through holistic, interdisciplinary care management. This is an excellent opportunity to make a meaningful impact while working remotely with a supportive clinical team.Position Details:Location: Remote (Orientation held in the Bronx, NY for 1 week)Schedule: Monday–Friday | 8:00 AM – 5:00 PM (1-hour lunch)Pay Rate: $39–$40/hourContract Length: LOA Coverage from 5/15/2026 – 9/21/2026 (subject to change)Position Overview:Responsible for overall care management and quality of care for participants. Uses specialized discipline-specific knowledge to review assessments of field staff and coordinate a holistic care plan that addresses all domains of care.Key Responsibilities:Participate in care planning meetings and represent the social work discipline as neededReview discipline-specific documentation for quality and address deficiencies with field staffMonitor field staff documentation of interventions and provide feedback or corrective action as neededConduct coaching sessions with field staff to support performance and complianceCommunicate regularly with field staff to coordinate a continuum of care aligned with members’ needs and goalsDevelop and manage holistic care plans to support optimal health and functional outcomesCoordinate with the interdisciplinary team (IDT) to arrange and authorize appropriate services (e.g., ADLs, IADLs, housing, meals, transportation)Implement care management interventions to help participants achieve their goalsCollaborate with team members, facilities, discharge planners, and home care coordinators during care transitionsDocument all services in compliance with organizational standards and federal/state regulationsCoordinate long-term care services (e.g., nursing homes, rehab facilities) as neededPartner with PCPs, specialists, and IDT members to address changes in participant conditions and adjust care plans accordinglyArrange or provide ongoing skilled services, authorizations, and periodic reassessmentsMonitor effectiveness of care plans and adjust services based on participant progress and outcomesEvaluate care plans and modify as needed to meet changing health needs and goalsEnsure cost-effective service delivery aligned with member needsMaintain up-to-date knowledge of healthcare practices, guidelines, and regulationsConduct staff trainings and competency evaluations as neededRecommend improvements to programs, policies, and proceduresFollow organizational policies related to disciplinary actions and collaborate with HR when neededAct within scope of practice and maintain required competenciesServe as an active member of the interdisciplinary team (IDT)Perform other duties as assignedEducationMaster’s Degree in Social Work from a program approved by the New York State Education Department (NYSED)LicenseActive, unrestricted New York State Social Work license and registration required:LMSW or LCSWExperienceMinimum of 2 years of administrative experience in one or more of the following:Certified Home Health Agency (CHHA)Managed CareLong-Term Home Health Care (LHCSA)Acute CareMedical-Surgical/Critical CareNursing HomeDiagnostic/Treatment ClinicCustomer service experience requiredManaged long-term care insurance experience preferredSupervisory experience preferredMinimum of 1 year working with a frail or elderly populationCandidates with less than 1 year must complete PACE-specific training upon hire
