Social Work Care Manager (LMSW/LCSW)**Training onsite in Bronx- then fully remote after training**Position OverviewWe are seeking an experienced and compassionate Social Work Care Manager Supervisor to join our interdisciplinary healthcare team. This role is responsible for overseeing care management activities, supporting field staff performance, coordinating holistic care plans, and ensuring high-quality, cost-effective services for a frail and elderly population. The ideal candidate will bring strong leadership skills, managed care experience, and a commitment to improving member outcomes through collaborative, patient-centered care.Key ResponsibilitiesParticipate in interdisciplinary care planning meetings and represent the social work discipline as neededDevelop, implement, and manage holistic care plans to support optimal health and functional outcomesCoordinate with interdisciplinary team (IDT) members to arrange and authorize appropriate services including ADLs, IADLs, housing, meals, and transportationCollaborate with PCPs, specialists, discharge planners, facilities, home care coordinators, and other providers during care transitionsMonitor the effectiveness of care plans and modify services based on participant progress, outcomes, and changing healthcare needsCoordinate long-term care services including nursing home placements, rehabilitation services, and community-based supportsReview discipline-specific documentation for quality assurance and address deficiencies with field staffMonitor staff documentation of interventions and provide ongoing coaching, feedback, and corrective action as neededConduct coaching sessions, staff trainings, and competency evaluations to support compliance and professional developmentCommunicate regularly with field staff to ensure continuity of care aligned with participant goals and needsEnsure all services and documentation comply with organizational standards and federal/state regulationsRecommend improvements to programs, policies, workflows, and care management proceduresSupport disciplinary processes in collaboration with Human Resources when necessaryMaintain current knowledge of healthcare regulations, best practices, and managed care guidelinesServe as an active member of the interdisciplinary team and perform additional duties as assignedQualificationsEducationMaster's Degree in Social Work from a program approved by the New York State Education Department (NYSED)LicenseCurrent, active, and unrestricted New York State Social Work license requiredLMSW or LCSW acceptedExperienceMinimum of 2 years of administrative or leadership experience in one or more of the following settings:Certified Home Health Agency (CHHA)Managed CareLong-Term Home Health CareLHCSAAcute CareMedical-SurgicalCritical CareNursing HomeDiagnostic and Treatment ClinicMinimum of 1 year of experience working with a frail or elderly populationCandidates with less than 1 year must complete PACE-specific training upon hireCustomer service experience requiredManaged Long-Term Care (MLTC) experience preferredSupervisory experience preferredBilingual language skills preferredScheduleMonday – Friday8:00 AM – 5:00 PM(1-hour lunch break)Ideal CandidateThe ideal candidate is a collaborative leader with strong clinical judgment, excellent communication skills, and experience managing complex care needs within a managed care or long-term care environment.
