Jobs locations: Morris Plains, NJ ( 1 Opening) Brick, NJ ( 2 Openings)Qualifications: Bachelor's degree or Master's degree in social work, psychology, counseling or a related field, and a minimum of two years of relevant experience in partial care or a supervised residential setting, working with the seriously and persistently mentally ill.Responsibilities: Responsibility to establish and maintain an ongoing therapeutic relationship and case manage up to 20 mixed risk level persons served. Conduct various psychoeducation groups and provide individualized case management sessions. Maintain up-to-date person's served records, including weekly progress notes and develop individualized recovery plans collaboratively with persons served. Provide personal support; crisis intervention and advocacy for needed services and entitlements.Skills/ Qualifications: Efficient oral and written communication, use of good decision-making abilities, conflict resolution skills. Recovery-focused approach to serving adults with mental illness.The case manager will be responsible for assigning, monitoring, and organizing cases for social work. They will be directly involved in the lives of their clients, and provide resources and assistance. Planning health services, working with social workers and doctors, and scheduling treatment or recovery is all in a day's work. The case manager must be highly organized, empathetic, compassionate, nonjudgmental, and eager to help vulnerable adults.Case Manager Duties and ResponsibilitiesAccessing client needs, and developing strategies to help the clientProviding support and resources for clients; good working relationship with support networks, government resources, and community resourcesResearching and referring client resourcesRespond to crisis situations that affect vulnerable or mentally unstable adultsEvaluating and studying programs, services, and resources for quality and client needsDetermining the correct course of action when helping clientsCaring for the well-being of clients and helping a wide range of clients, including people with serious illnesses, addictions, disabilities or who have been neglectedAdvocating and raising awareness on behalf of clients and the needs of the community and local servicesAssisting clients by providing and facilitating various psycho-education groups that focus on clients mental health needsCase Manager Requirements and QualificationsPossesses superb written and spoken communication skillsExcellent interpersonal skills with colleagues, community leaders, policymakers and othersExcellent time management skills; organized and able to prioritizeComfortable in a fast-paced environment with multiple casesAble to organize and manage necessary paperwork and documentationSelf-directed and able to work without supervisionComfortable with building personal relationships and dedicated to helping othersEmpathetic and supportive with mentorship and leadership skillsBachelor's degree in social work, psychology, sociology, healthcare or medical and related fields; Master's degree in social work preferredProficient computer skills, including Microsoft Office Suite (Word, PowerPoint, and Excel)Benefits to working with us:PTO (major holidays)Pay depending upon experience
