FP & IM Support NP/PA openings in Central Western OR (open to new grads)

Great group in Central Western Oregon about 1hr 20mins from Portland has Family NP/PA and Internal Medicine NP/PA support openings. M-F, forty (40) work hours per week, including a minimum of twenty-four (24) hours availability for patient contact per week.Providing same-day or next-day access for patients with urgent, low acuity care needs;Providing interim co-management of chronic illnesses (titrating medications, interval routine follow-up care)Conducting, as needed, CMS Annual Wellness Visits; and,Meeting on a regular basis with internal medicine providers to review patient charts, collaborate on patient cases, and discuss workflow and expectations.The role will support up to, but not exceed, four (4) primary care providers.
