Job Title: Licensed Vocational Nurse (LVN) - Home Health Job Type: Pay Per Visit Locations: North-East Houston, TX / North-West Houston, TX / Baytown, TX About the Role: Our client is seeking experienced and dependable Licensed Vocational Nurses (LVNs) to join their growing home health team. Ideal candidates will bring strong clinical skills, professionalism, and a collaborative, patient-centered approach. This role involves providing skilled in-home nursing services, including assessments, wound care, and specialized procedures. Candidates must be comfortable traveling within a 40-mile radius of their home address. Candidates must have prior home health experience and a clear understanding of how home health operations work. Key Responsibilities: Perform comprehensive body assessments and document findings within required timeframes (48 hours). Provide wound care, including wound VAC management. Conduct phlebotomy (blood draws) as ordered. Perform foley catheter insertion and care, PICC line care, and tracheostomy care. Deliver patient-centered care under the supervision of an RN or Case Manager. Communicate effectively with patients, families, and the interdisciplinary team. Maintain timely, accurate documentation compliant with agency and regulatory guidelines. Adhere to professional appearance standards and company policies. Qualifications: Active LVN license in the state of Texas. Minimum 1 year of clinical experience, preferably in home health or skilled nursing. Strong clinical judgment and assessment capabilities. Reliable transportation and willingness to travel up to 40 miles. Proficiency in wound care, PICC line management, tracheostomy care, and catheter care. Ability to work independently while maintaining communication with the care team. Soft Skills & Expectations: Strong communication and teamwork abilities.
