Patient Care Coordinator - Remote

*Location:* Remote - PST or MST Time Zones*Job Type:* 2Month Contract (with possibility of extension based on business needs)We're looking for a compassionate and detailoriented *Patient Care Coordinator* to support patients throughout their care journey. This role serves as a central point of coordination between patients, providers, facilities, pharmacies, and insurance partners to ensure timely, accurate, and wellcommunicated care.This is a great opportunity for healthcareexperienced professionals who thrive in fastpaced, patientfacing environments and are comfortable working remotely with high volumes of coordination and communication.*Key Responsibilities*Serve as the primary point of contact for patient questions related to referrals, imaging, lab work, medications, and treatment plansCollaborate with healthcare providers, clinics, and external facilities to coordinate patient care servicesManage referrals, tests, procedures, and followups to ensure care is delivered efficiently and on timeCoordinate medication and authorization information with pharmacies and insurance providersCommunicate updates to patients and healthcare facilities regarding lab results, imaging status, and service timelinesSchedule appointments and manage scheduling requests using appropriate resources for all appointment typesApply critical thinking to identify issues, evaluate options, and implement effective solutionsMaintain accurate and uptodate patient records, demographics, and documentation to support continuity of care and complianceMonitor patient progress and satisfaction to identify gaps or opportunities for improvementCommunicate with empathy, professionalism, and clarity across phone and written channelsPerform additional duties as assigned to support care coordination needs*Qualifications* *Required*High school diploma or GEDProficiency with Google Workspace, Microsoft Office, and standard computer systemsStrong organizational, communication, and documentation skillsAbility to work independently in a remote, highvolume environment*Preferred*2+ years of experience in healthcare or customer service settings1+ year of experience working with EHR systems (athenaOne preferred)Understanding of medical terminology and healthcare insurance processesPrior remote experience handling inbound and outbound calls*Job Type & Location*This is a Contract position based out of Salt Lake City, UT.*Pay and Benefits*The pay range for this position is $23.00 - $23.00/hr.Eligibility requirements apply to some benefits and may depend on your jobclassification and length of employment.
