We are located in the beautiful City of Cloverdale, northern rural Sonoma County.Tasks/Duties Include:Works closely with primary care providers to assess patient needs and develop appropriate support and/or interventionsBehavioral services include but are not limited to; individual therapy, behavioral health education, support groups, information on other community resources and linkages to other resourcesProvides case management support as necessary to ensure that client's medical, mental health and social service needs are being addressed and metProvides behavioral health consultation to primary care providers and support staffWorks with medical staff to manage crisis situationsParticipates with Director of Behavioral Health in the development and delivery of trainings on behavioral health topics for medical, dental, and support staffCompletes and maintains patient records that meet applicable legal and ethical standards and the requirements of AVHMaintains high ethical standards, especially confidentiality, as required by AVH, the profession, state and federal regulationsParticipates in applicable AVH quality assurance proceduresAttends behavioral health team meetings, relevant agency meetings, and supervision as directedSeeks clinical and administrative supervision as neededParticipates in ongoing trainingRequirementsQualificationsLMFT with a valid, unrestricted California licenseCurrent CPR/BLS certificationExperience:One-year experience providing similar services preferredExperience working in an integrative primary care facility preferredExperience working with people from various cultures and backgrounds preferredKnowledge and Skills:Bilingual Spanish and English fluency requiredKnowledge of compliance issues within the community health center environmentKnowledge of HIPPA policiesKnowledge of incident and mandated reporting requirements and systemsCompletes assigned policy review itemsProficiency with Microsoft Office applications including Outlook, Word, Excel and PowerPointStrong interpersonal skills; ability to be sensitive with persons of various social, cultural, economic, and educational backgroundsAbility to work independently and as part of a teamGood judgement, problem solving and decision-making skillsStrong written and verbal communication skillsBenefitsJob Types: Full-time, Part-timePay: $55.00 - $70.00 per hourExpected hours: 20 - 40 per weekBenefits:401(k)401(k) matchingDental insuranceEmployee assistance programFlexible scheduleFlexible spending accountHealth insuranceLife insurancePaid time offProfessional development assistanceRetirement planTuition reimbursementVision insuranceMedical Specialty:Addiction MedicinePsychiatrySchedule:10 hour shift4 hour shift8 hour shiftMonday to FridayAbility to Commute:Cloverdale, CA 95425 (Required)
