A capitated medical practice receives a fixed, predetermined payment for each patient assigned to them over a specific period, typically on a per-member, per-month basis, regardless of how many services the patient uses.The care model is built around value-based, preventive medicine with a focus on proactive care, chronic disease management, and strong physician-patient relationships. The organization partners with Medicare Advantage health plans to deliver coordinated care designed to improve outcomes and reduce hospitalizations.
