Salary: $22.00 - $30.00 per hourHome & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.Overview of the RoleWe want to hire bachelors prepared Social Workers (or another related field such as Gerontology) who will work with a Care Management Team (CMT) consisting of Masters or Bachelors prepared Social Worker and Registered Nurse team to provide comprehensive case management services to target population.Who You Are:You are a Master's or Bachelors Prepared Social Worker (BSW) passionate about making a difference in the lives of individuals.You are detail oriented, computer literate, and want to work as part of a management team supervising the care of clients who are receiving home and community-based services.You are good at problem solving, knowledgeable about community resources, and enjoy helping clients solve social determinants of health care problems.You want a workplace that values the significant differences Social Workers can make to the health and welfare of clients.You like working 8am-5pm on weekdays with no weekend work.Responsibilities:Assists the team with finding community resources (i.e., housing; medical equipment and supplies; primary care physician and specialists; essential items like food, clothing, and household goods; medical transportation; and caregivers, etc.) needed to meet the participants needs.Engages in advocacy efforts to ensure participants receive timely medical services to remain safe in the communityResponds timely to questions and enquiries from CMT, participants, and other entities for smooth patient care coordinationProvides education to participants so they can better understand the benefits of services and community supports.Assists participants with applications (i.e., Medi-Cal; IHSS; Calfresh; and Regional Center, etc.). Explains eligibility requirements, coordinates transportation for participants and other basic needs of supportDocument all consumer interaction and maintain timely reporting and documentation requirementsObserves participants behavior; reports significant changes to supervisor or team; documents appropriatelyDaily case conference with CMT about shared participantsPerforms other related duties as assigned.Essential Requirements:Master's degree or Bachelors degree in Social Work or another related fieldSkilled in operating a personal computer utilizing a variety of standard softwareValid California Drivers License and eligible to be insured under our liability policy with a clean DMV reportYour own vehicle and proof of current auto insuranceCPR certificate required within 30 days of hirePreferred Skills, Knowledge and CertificationsKnowledge of basic problems and needs of people diagnosed with a physical disability or emotional disturbanceKnowledge of community and supportive servicesSkilled in assessing and prioritizing multiple tasks, projects, and demandsSkilled in working within deadlines to complete projects and assignmentsSkilled in establishing and maintaining effective working relations with co-workers and other community providersExperience with electronic medical recordsLocation and Service Region:This is not a remote role, and you are required to work from our Sacramento office.You will need your own car and be able to travel to clients homes in Sacramento County and the greater surrounding area. Work is performed in a community and office environment.PPE is provided to all employees.Transporting clients with personal vehicle is prohibitedSchedule:Full time, 40 hours per week, from Monday through Friday from 8am to 5pm with an hour for lunch. No weekend work.Currently 20% of the time is related to direct home visits, 80% of the time is telehealth video visits and or calls and documentation in the office.Physical Requirements:Job offer is conditional upon successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirement for this role specifically includesSits, stands, bends, lifts, walks, and moves intermittently during working hoursAble to lift to 35 pounds without assistanceAble to drive intermittentlyCompensation:Pay range is $22 to $30 per hour.
