The interdisciplinary team, in consultation with the patient/family and physician(s), collectively assesses, coordinates, develops and implements a plan of care (POC) that is directed towards teaching the patients/families/caregivers how to manage the illness at home, assist the patient and family through the rehabilitative process, and teaches them how to return to the highest level of functioning possible to maintain independence in their own environment. This position may be designated as the Director of Nursing and/or Alternate Administrator for regulatory purposes.This position supervises and directs the activities of various levels of assigned nursing and ancillary personnel utilizing both professional and supervisory discretion and independent judgment, while practicing within the State, facility and regulatory guidelines.The value you'll bring The expertise and experiences you'll need to succeed:Minimum qualifications: Associate's degree in nursing or other healthcare related field. Three (3 ) years of experience in home care, Hospice or related healthcare field. Clear/active Registered Nurse Maintain current, valid Driver's License in good standing with a safe driving record and a reliable vehicle. Maintain valid personal automobile insurance. Basic Life Support certification. Supervisory Responsibilities:Supervises staff providing direct patient care such as home health aide, RN, LPN. May also supervise other office staff who support the delivery of care as per organization chart.Preferred Qualifications:At least one (1) year of experience in clinical supervisor or quality review activities.
