Job TypePart-timeDescriptionHome Health Aide / Certified Nursing AssistantReports to: HHA AdministratorJOB DESCRIPTION:The Home Health Aide (HHA) role requires compassionate, dependable individuals to provide high-quality, patient-centered care in the home. This role is responsible for assisting clients with activities of daily living (ADLs), promoting independence, and supporting overall well-being under the direction of a Registered Nurse. Home Health Aides play a vital role in delivering care that allows clients to remain safe, comfortable, and dignified in their homes, while serving as a trusted extension of our care team.RESPONSIBILITIES:Personal Care & Daily Living Support:Assist clients with activities of daily living (ADLs), including:Bathing, grooming, and personal hygieneDressing and toileting assistanceMobility, transfers, and ambulation supportProvide compassionate care while maintaining client dignity and respectHome Support & Companionship: Assist with light housekeeping tasks, including:Laundry, meal preparation, and basic cleaningProvide companionship and emotional support to reduce isolation and improve quality of lifeEncourage engagement in daily routines and activitiesCare Plan Execution: Follow the established plan of care as directed by the Registered Nurse or clinical teamObserve and report changes in client condition, behavior, or environmentSupport clients with reminders for medications and appointments (as permitted)Communication & Team Collaboration: Maintain clear and professional communication with clients, families, and care team membersParticipate in care coordination by providing timely updates and feedbackRepresent the agency with professionalism and compassion in every interactionDocumentation & Compliance: Complete accurate and timely visit documentation per agency standardsFollow all HIPAA, infection control, and safety protocolsAdhere to all state regulations and agency policiesTravel Requirements: Travel to client homes within the assigned service areaWillingness to drive up to 45-60 minutes between shifts or visits as neededManage schedule efficiently to ensure reliable and consistent client careRequirementsQUALIFICATIONS: Current Home Health Aide (HHA) certification (or CNA if applicable per state requirements)Experience in home care, home health, or senior care settings preferred6-12 months of caregiving experience preferredCompassionate, dependable, and service-oriented mindsetAbility to follow care plans and work independently in a home settingStrong communication and interpersonal skillsValid driver's license, reliable transportation, and current auto insuranceWillingness and ability to travel within the service area, including some extended drive timesCurrent CPR certification preferred
