Nurse Manager – Care Management ProgramWe’re hiring an experienced Nurse Manager to lead an established ACO Care Management Program serving pediatric, adult, and maternal populations.This role provides hands-on leadership for a multidisciplinary team of RN Care Managers, Behavioral Health Care Managers, and Community Health Workers, with a strong focus on care coordination, population health outcomes, and regulatory compliance.Key responsibilities include: • Leading daily care management operations • Ensuring compliance with MassHealth and CMS requirements • Monitoring program metrics and improving performance • Coaching and developing a clinical care management team • Partnering with population health leadership on improvement initiativesWhat we’re looking for: • BSN with active Massachusetts RN license • 3–5 years of management experience in care management or managed care • Experience supporting Medicaid and high-risk populations • Strong leadership and data-driven decision-making skillsThis is a full-time leadership role with the opportunity to make a real impact in population health and care coordination.
