Exciting Opportunity for Inpatient Case Management Social Worker (LMSW or LCSW) Job Description: - Position: Inpatient Case Management Social Worker - Location: Acute Hospital - Shift: Day 5x8-Hour (08:30 - 17:00) - Years of Experience Required: Minimum 2 years preferred - Certifications Required: AK License as LMSW or LCSW (Pending License Accepted) Key Responsibilities: - Provide psychosocial/emotional support to patients and families. - Conduct psychosocial assessments and develop person-centered discharge plans. - Assist with resource navigation and coordination of care, ensuring compliance with policy and regulations. - Screen for suicidality and engage in safety planning as appropriate. - Coordinate with care team and RN Case Managers to implement safe and ethical discharge plans. - Provide brief clinical interventions as necessary. Preferred Skills and Experience: - Experience in Acute Hospital settings (specifically in Emergency Departments, ICU, L&D, Medical Surgical units, etc.) - Knowledge of major disease processes including Cancer, CHF, Dementia, Diabetes, and more. - Ability to educate and support patients/caregivers for effective discharge planning. - Familiarity with community-based service programs and protective services. Additional Information: - Weekend Coverage: Every other weekend - Daily Patient Census: 10-20 - Patient Diagnoses Experience: Preferred experience with Neonates, Infants, Pediatrics, Adults, and Geriatrics Special Procedures/Unit Details: - Care Management Social Work is a consult-based service with a focus on collaborative care. - Clinicians work closely within three areas: Emergency Department, Children's, and Adult teams. - Engage in care conferences and document all relevant information to facilitate care coordination.
