This opportunity allows you to work in a patient-focused environment where the goal is to help participants maintain independence, improve mobility, and enhance their overall quality of life.This role is ideal for a therapist who enjoys working with the geriatric population and values being part of an interdisciplinary team that focuses on coordinated, individualized care.Position OverviewThe Physical Therapist is responsible for evaluating participants and developing individualized treatment plans designed to support functional mobility and safety. Care may be provided at an acute, restorative, or maintenance level depending on participant needs. Treatment plans are incorporated into the participant's overall plan of care and carried out in collaboration with physicians and the interdisciplinary care team.Key ResponsibilitiesConduct physical therapy evaluations and determine rehabilitation needs for program participantsDevelop and implement individualized treatment plans based on physician orders and participant goalsProvide therapeutic exercise, functional mobility training, and other appropriate treatment interventionsCommunicate participant progress and recommendations with the interdisciplinary care teamEducate participants, caregivers, and family members on safety, positioning, and mobility strategiesProvide guidance on the proper use and care of durable medical equipmentDelegate appropriate treatment activities to Physical Therapist Assistants and rehabilitation support staffParticipate in interdisciplinary care meetings to support coordinated participant careConduct home visits or educational sessions when indicated in the plan of careMaintain timely and accurate documentation in accordance with program policies and regulatory guidelinesAssist with ordering, maintaining, and tracking durable medical equipment inventoryParticipate in quality improvement initiatives and peer collaboration within the rehabilitation teamQualifications RequiredBachelor's Degree in Physical TherapyGraduation from an accredited physical therapy program approved by the American Physical Therapy Association or equivalent organization recognized by CMSActive Physical Therapist license in the state of FloridaAt least one year of experience working with older adults or medically complex populationsPreferredTwo or more years of experience practicing as a Physical TherapistMaster's or Doctorate in Physical TherapyBilingual skills are a plusCompensation and Benefits$5,000 sign-on bonus for qualifying candidatesHourly pay range of $40 - $52 based on experienceComprehensive benefits packageOpportunities for professional development and advancementSupportive team environment focused on quality patient careBenefits:401(k) matchingDental insuranceDisability insuranceEmployee assistance programHealth insuranceHealth savings accountLife insuranceOpportunities for advancementPaid time offRetirement planVision insurance#CMAllPandoLogic.
