Health Coach (RN) – Remote (PA, NJ, DE)Location: Fully Remote – Must reside in PA, NJ, or DELicense: Active Pennsylvania RN license required (or Compact license including PA)About the RoleWe are seeking an experienced Registered Nurse (RN) Health Coach to provide telephonic health coaching and case management services to members across the continuum of care — from health promotion and disease prevention to complex care. In this role, you will use clinical expertise and motivational interviewing to help members improve health outcomes, navigate the healthcare system, and achieve sustainable lifestyle changes. You will partner closely with members, families, and healthcare providers to develop personalized care plans and remove barriers to care.Key ResponsibilitiesProvide telephonic health coaching and case management to membersConduct health assessments and develop individualized care plansSupport members with chronic conditions, acute concerns, and preventive careIdentify barriers to care and help members achieve health goalsCollaborate with providers to coordinate careEducate members and connect them with appropriate health resourcesMaintain accurate documentation and ensure quality standardsQualificationsActive PA RN license (or Compact including PA)3+ years of clinical nursing, case management, or disease management experienceCCM certification preferredStrong communication, coaching, and clinical assessment skillsTelephonic care management experience preferredAbility to work flexible hours including occasional late shifts
