Manage acute and chronic conditions, including medications Review and follow up on lab, X-ray, and referral results Coordinate patient health education and follow-ups Participate in on-call and phone triage rotationsMedical Records: Maintain accurate and timely chart documentation Review and sign off on lab results and patient dataConsultations and Referrals: Consult with the Medical Director for complex cases Refer patients to physicians, specialists, or support services as neededTeamwork: Join clinician peer reviews and quality improvement meetings Participate in daily huddles with the care team Attend monthly staff meetings Requirements:Education: Accredited Nurse Practitioner Program graduate License: Valid California Nurse Practitioner License Certifications: Current CPR/BLS certification Skills: Bilingual (English/Spanish) preferred, strong patient and staff interaction skills by Jobble
