HOUSING OPTIONS & GERIATIC ASSOCIATION RESOURCES, INC.JOB DESCRIPTION: CASAC-Case ManagerPosition Summary: The CASAC Case Manager provides direct services to residents in a supported housing program funded by the New York State Office of Mental Health (OMH). This program serves individuals with mental health diagnoses who are living independently in the community. The Case Manager supports residents in achieving recovery, housing stability, and personal wellness through individualized service planning, harm reduction strategies, and supportive counseling. In addition to providing consultation and support to the broader resident population, the Case Manager will also maintain a small caseload of residents for whom they provide direct case management services.Key Responsibilities:• Conduct initial and ongoing assessments of residents' mental health, substance use history, and functional capacity.• Develop, implement, and regularly review individualized service plans in collaboration with residents, focusing on recovery, housing retention, and personal goals.• Provide short-term counseling and crisis intervention related to substance use and mental health.• Educate residents and staff on relapse prevention, harm reduction, and recovery models.• Link residents to long-term behavioral health services, including outpatient therapy, psychiatric care, and substance use treatment as needed.• Maintain accurate and confidential documentation including progress notes, assessments, and service plans. • Facilitate support groups focused on wellness, recovery, and independent living skills.• Assist residents with life skills development such as budgeting, medication management, and navigating community resources.• Conduct periodic home visits to assess living conditions and provide in-home support.• Maintain a small caseload of residents for whom the Case Manager provides direct services, including case planning, referrals, and advocacy.• Participate in interdisciplinary team meetings and case conferences.• Maintain a current resource directory of mental health and substance use treatment providers.• Share on-call responsibilities for emergency support during evenings and weekends.• Collaborate with the Program Director to develop and revise program policies and procedures in alignment with OMH guidelines.• Perform other related duties as assigned.Minimum Qualifications:• Bachelor's Degree in Human Services, Psychology, Social Work, or related field.• CASAC certification or trainee status required.• Minimum of 3 years' experience working with individuals with mental health and/or substance use disorders.• Strong interpersonal skills and ability to build trusting relationships with residents.• Experience in supported housing, independent living programs, or community-based mental health services preferred.
