Park Nicollet is looking to hire a Registered Nurse to join our Home Care team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.Position Summary:The Home Care registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.The nurse is responsible to improve or maintain patient's health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting.The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery.Central RegionWork Schedule: M-F 8a-4:30p. Weekends approximately every 6th/7th weekend. Monthly on-call and quarterly weekend on-call. 2 holidays/yearRequired Qualifications:* Education, Experience or Equivalent Combination:* Work experience as a RN for at least two years* Licensure/ Registration/ Certification:* Current MN Registered Nursing LicenseKnowledge, Skills, and Abilities:Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.Proven proficiency in the operation of the following: Personal Computer, Telephone, Fax Machine, Photocopier, Printer, and electronic medical record.Preferred Qualifications:Experience in home care and or hospice desirable.Must be able to function independently with minimal supervision.High level of sensitivity, assessment skills, technical-skills and communications skills.Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.Benefits:Park Nicollet offers a competitive benefits package (for eligible positions) that includes medical insurance, dental insurance, a retirement program, time away from work, insurance options, tuition reimbursement, an employee assistance program, onsite clinic and much more!
