Community Health Worker- Case Management

Schedule: Monday-Friday 8 hour days- position serves both Birch Grove and Bryson Center Overview As a member of the Case Management Team, the Community Health Worker (CHW) is a trusted member of and/or has an unusually close understanding of the community served. Through the provision of culturally and linguistically appropriate services, the CHW will work to improve the health of those with behavioral health needs where by equitable opportunities are ensured for health access, barriers to care are reduced, and health disparities are addressed. The CHW helps clients and their families/significant others navigate and access community services and needed resources to improve health. The CHW supports clients through an integrated approach to health and community outreach. The CHW mentors, educates and advocates for individual health needs. The CHW serves as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The CHW will help build community awareness of and access to CCBHC integrated health services through a range of activities such as outreach, education and advocacy. Utilizes Feedback Informed Treatment (FIT) in delivery of client services. Options provides on the job training and supervision for FIT. Responsibilities Responsible for establishing trusting relationships with clients and their significant others while providing general support and encouragement. Provide ongoing support and engagement, using motivational interviewing and person-directed goal setting. Assist individuals in identifying and navigating to and through various health and social resources that will help clients achieve better health outcomes. Collaborate with clients, clinical staff across a variety of areas within Options, and community partners to empower clients in securing the services and support they need. Follow-up with clients via phone calls, home visits and visits to other settings frequented by those needing care. Help individuals connect with transportation resources and provide appointment reminders as needed. Attends client appointments and advocates for client needs as needed. Work collaboratively with medical providers and clinical staff to ensure clients have comprehensive/coordinated care plans. Assists clients and/or teaches the skills necessary for clients to access community resources. Provides timely (completed same day of services) and accurate documentation. Coordinates care with medical providers, housing and residential programs, sobering centers, substance abuse treatment agencies, CRC, senior and disabled services, and other service providers. Qualifications Must be currently certified, or eligible to become certified within 6 months of hire, by the State as a Community Health Worker and listed in the Traditional Health Worker (THW) registry https://traditionalhealthworkerregistry.oregon.gov/. Education and experience providing behavioral health and/or social supports to vulnerable populations and knowledge of local resources preferred. Demonstrated ability to interact positively, and in a culturally appropriate manner, with a wide range of communities including those historically underserved. Member of and/or unusually close understanding of the communities served (for example: those with behavioral health needs, BIPOC community, LQBTQAI+ community, people without housing, Veterans and active duty military, Child Welfare involved families.) Candidates must be able to use e-mail, compose documents, save and locate documents electronically, and learn to use our Electronic Health Record (EHR) system. Proficiency in Word and other MS Office applications preferred. Must pass state-required background and DMV checks; possess a valid OR driver's license and a satisfactory driving record to transport clients. Candidate must be able to work independently and flexibly, under general supervision. Possess mature attitude and sensitivity to the needs of people who have a chronic mental illness; understanding of HIPAA laws regarding confidentiality of privileged information; ability to follow verbal and written instructions; ability to work independently and with staff as agency needs dictate.
