Assistant Director of Community Health

Job Description Job Summary:This position provides direction for our community health programs and is responsible for program performance, contract compliance, staff leadership, and alignment with the agency&#39;s mission and strategic goals. Ensures high-quality service delivery, financial sustainability, operational efficiency, and the integration of person-centered principles and best practices across programs to achieve optimal outcomes that allow AAAPP to continue to serve seniors and caregivers in our planning and service area.Duties and Responsibilities:Responsible for day-to-day operations of SHINE/SMP/MIPPA, Veterans Directed Care, GUIDE, and Senior Community Health programs, and ensures that all contractual requirements are met, at a minimum.Supervises the SHINE/SMP/MIPPA Program Manager/Liaison, VA Program Manager, and GUIDE team to ensure they are motivated, valued, and held accountable.Oversees implementation of the GUIDE program to ensure all CMS/CMMI requirements are met and in accordance with Mobile Physician Services partnership agreement.Tracks Title VII/ Abuse, Neglect and Exploitation quarterly deliverables in coordination with the Outreach Manager, under the direction of the COO.Accept referrals with unmet needs to review for potential assistance.Coordinate diaper distribution for the Diapers for Dignity program.Works closely with the CFO and COO to ensure the above-referenced program budgets are being spent in accordance with all contractual obligations to ensure that older adults, adults with disabilities, and caregivers are getting the services they need.Reviews Surplus/Deficit Reports for the above-referenced programs monthly with Finance and provides a summary to the COO.Keeps COO abreast of all internal and external emerging issues so that they can be resolved on time.Works with Program Managers to ensure that all monthly, quarterly, and annual reports are prepared accurately and submitted on time.Participates in conferences, workshops, and similar activities relevant to the assigned functional area of responsibility.Keeps abreast of federal/state regulations, program innovations, and gerontological research.Participates in SHINE public awareness and outreach events, as needed.Compiles with all security and confidentiality regulations.Participates in emergency preparedness activities consistent with DOEA and agency COOP/disaster plans.Leads by example in fostering a workplace culture that reflects the agency's core values, ensuring a supportive, inclusive, and mission-driven environment where staff are empowered, collaboration is encouraged, and excellence in customer service is prioritized.Approves timesheets for supervised staff and their teams.Any other duties as assigned by the COO.Education:Bachelor's degree in gerontology, social work, or a related field from an accredited college or university, or at least 10 years of progressive supervisory experience in the aging field.Skills/Qualifications:Expert knowledge of federal and state-funded programs, ability to analyze comprehensive problems; ability to organize data effectively, test its validity, and draw sound conclusions from it. Previous experience working with DOEA programs required. Excellent written and oral communication skills. Excellent computer skills, including Microsoft Word and Excel. The ability to establish and maintain effective working relationships with community groups, preferably the elderly, governmental agencies, and other staff members.Special Requirements: Must pass DOEA Level II criminal background screening and annual conflict of interest form. Must sign Medicaid Attestation Payroll Form per Department of Elder Affairs/AAAPP requirements.
