Clinical Weekend Supervisor, VNA of Cape Cod
The Supervisor has the responsibility for organizing, coordinating, implementing, and evaluating the care management as well as the safety of patient care in designated area(s).
Description:
1. Ensures consistent evidence based practice standards are developed and upheld.
2. In collaboration with the Human Resource Department, assist Clinical Team Managers with the review of applicants, interviews and hiring process as needed.
3. Assists Clinical Team Managers with orientation and precepting as needed.
4. Assists Clinical Team Managers to coach, support, and monitor progress of their direct reports.
5. Organizes, directs, coordinates and evaluates the activities related to patient management. Assess this data utilizing tools and reports designed and developed by Business Analyst as well as or clinical software (SHP and Delta).
6. Assists Clinical Team Managers with patient care management through Interdisciplinary Team Conferences and communication.
7. Ensure accurate diagnostic coding, and OASIS data congruent with patient profile and plan of care.
8. Ensure staff are meeting productivity.
9. Provide clinical expertise and serve as a resource to staff (including CHHA's) and community.
10. Assists the Clinical Team Managers with team meetings as needed.
11. Routinely rotates through clinical management on-call responsibilities to ensure proper patient/staff safety.
12. Ensure weekend/holiday on-call and staff coverage to meet agency need.
13. Provide direct care as indicated by agency need.
14. Assist Clinical Team Managers with performance evaluations as needed.
15. Work in collaboration with Human Resources to discipline clinicians through education, corrective action plans, verbal/written warnings and or discharge as indicated.
16. Ensure compliance with all governmental and DPH clinical guidelines, conditions of participation, grant requirements and JCAHO accreditation.
17. All other duties as assigned.
Qualifications:
Current RN license in Mass.
CPR is required.
BSN or MSN preferred.
Minimum three (3) years clinical experience including two (2) in homecare/community health required.
Computer skills preferred.
