Job RequirementsPOSITION SUMMARY:The Hospice RN oversees the development, implementation and evaluation of the plan of care in collaboration with the interdisciplinary team to create the most positive Hospice experience for the patient and family.PRIMARY ACCOUNTABILITIES:Quality/No Harm:Verbalizes, demonstrates and documents nursing care, change in condition, treatments and procedures in compliance with Health First/Hospice Policy, Federal/State Regulations and National Safety Patient Goals. Does Well = meeting of goals as per individual audits/action plan audits.Demonstrates effective medication administration and appropriate delegation and supervision of the CNA to include the care planning process and supervisory oversight. Does Well = compliance with CNA care plan audit and CNA supervisory audit.Implementation of consistent nursing practices across the 4 levels of Hospice care (Routine, Inpatient, Respite and Crisis/Continuous Care).Customer Experience:* Within the interdisciplinary team, provides patient, family and/or caregiver educational material as directed by the plan of care.* Provides service to patients and families with sensitivity and respect for their needs, expectations, age and cultural, spiritual, physical and individual needs.Stewardship:Demonstrates fiscal responsibility with medication management, supplies and other resources.Ensures all patient transfer information from one level of care to the next is accurately and completely documented for utilization by the billing department to meet deadlines as per corporate finance.Time management skills to prioritize and manage diverse but equally important tasks or responsibilities while utilizing resources correctly.Work ExperienceQUALIFICATIONS REQUIRED:Current Florida RN license and endorsement permit.Graduate of an Accredited School of NursingMinimum of two years' experience in Oncology, Medical/Surgical or Home Health First, Inc.Must hold current AHA BLS Healthcare Provider Completion Card prior to start date and maintain.Successful completion of Level II background screeningExperience with Palliative Care/End of Life issues helpful.Completes orientation checklist within three months of employment.Knowledge of regulatory guidelines (state and federal).Ability to maintain a valid Florida driver's license and insurance.Must have reliable transportation.Working conditions:Varied environments for patient care visits.*PHYSICAL DEMANDS:Must be able to speak/understand written and verbal instructions in the English language.Must have visual acuity and hand-eye coordination to perform clinical tasks.Must have corrected vision and hearing to a normal range.Must be able to lift, turn, pull, push or ambulate patients/equipment.Must be able to lift up to 40 lbs. unassisted.Must have the ability to perform a complete physical assessment on patients.Ability to sit or stand for an extended period of time (up to four hours).Potential for exposure to bio hazardous waste and known or unknown diseases.Flexibility in work schedule to cover other areas of Hospice services.(Field program to cover call and HH to cover continuous care).MENTAL DEMANDS:Must have ability to work in often stressful environments with patients/families at various ages and levels of understanding.Must have the ability to concentrate on multiple priority activities.Must have critical thinking and prioritization skills.Must have emotional stability to cope with loss and end of life issues frequently.BenefitsABOUT HEALTH FIRSTAt Health First, diversity and inclusion are essential for our continued growth and evolution.
