What You'll Be Doing You'll be conducting in-home assessments for NY Medicaid patients to evaluate their health status, care needs, and eligibility for long-term home care services. This is assessment and documentation work — you're not managing an ongoing patient caseload. Three days a week you're working remotely, two days you're out in the field.
