With locations throughout the state, they offer services in 18 hospitals and more than 350 outpatient locations.Position Details:? Full-time or Part-time options available with flexible schedules!? Locations: 1 Physician needed for Appleton and 1 Physician needed for Berlin/Oshkosh? The team consists of 5 Physicians and 8 APCs? On-Call by phone only? Patient census: 8-12? Support staff includes RNs and schedulers who help to coordinate provider visits, communicatewith facilities, and triage phone calls.? 27 Facilities total between skilled nursing and assisted living facilities? Large opportunities for growth such as Medical Director roles in the nursing homes that we serveAbout the RoleThe Physicians have excellent support staff,within the department itself and the organization overall. The APC and Physicians collaborate to provide high quality care for the patients we serve. The Physician/APC team is a mix of talented, young and experienced, individuals. The team is innovative andaccepting of new ideas. Every day there is an opportunity to come alongside of the facilities we serve to improve on their quality. Team based care model. Ideally looking for a Physician who is an independent worker-very mobile, and comfortable with remote type of work within the facility.Benefits/Compensation:? Base Salary: $287,000? Eligible to earn up to $30,000 value-based incentive? Sign on Bonus: $40,000 forgiven over 3 years? PTO: 35 days? CME: $4,500?
