This position is located in our Home Health department.Shift: On call - Will be discussed in the interview.What you get to do everyday!This position is responsible for the oversight of home health patients individualized plan of care from admission to discharge. You will:Provides services in accordance with the plan of careMakes the initial evaluation visit and regularly reevaluates the patient's nursing needsInitiates the plan of care and necessary revisionsProvides those services requiring substantial specialized nursing skillsInitiates appropriate preventive and rehabilitative nursing proceduresPrepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policyCoordinates servicesInforms personnel of changes in the condition and needs of the patientCounsels the patient and family/significant others in meeting nursing and related needsParticipates in and presents in-service programsUnderstands and adheres to established Agency policies and proceduresProcesses orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summariesDetermines the amount and type of nursing needed by each individual patientRefers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Social Worker those patients requiring their specialized skillsSupervises and teaches other nursing personnelConducts patient care conferences on patients assigned to his/her careParticipates in peer review and Quality Assurance and Performance Improvement as assignedGives total patient care as neededTakes on-call duty nights, weekends and holidays, as assignedCompletes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.Appropriately utilizes ICD-10 codesAdheres to Federal, state, and accreditation requirements including Medicare and Medicaid regulations.Adhere to infection control, standards of care and patient privacy standards.Performs others duties as assignedMay cross train for Hospice patients.Collaborate effectively across organizational boundaries.Adheres to the core values and standards of the organization.RequirementsGraduate from an accredited nursing programTwo or more years of direct work experience in healthcare.Current RN Washington State licenseCurrent BLS or complete within 60 days of hireOral and written fluency in the English language.
