Team Supervisor Job Category: Direct Service Requisition Number:TEAMS006256 Full-Time On-site Outpatient Clinic, ACCS Springfield, MA,USA The Center for Human Development, (CHD) is seeking a Team Supervisorfor our Adult Community Clinical Services (ACCS), located inSpringfield, MA. ACCS is funded through the Department of Mental Healthand is a multidisciplinary outreach approach for persons served in needof psychiatric and other services living independently in the community.This involves active engagement including assertive outreach, consistentassessment and treatment planning, skill building, symptom management,addiction treatment, and behavioral and physical health monitoring andsupport. YOUR ROLE AS A TEAM SUPERVISOR: ACCS teams include licensedclinicians, RNs, housing specialists, substance use specialists, directcare staff, and peer/family specialists. The Team works collaborativelyto provide person-centered, evidence-based services in support ofparticipant\'s treatment goals. The Team Supervisor assists participantsin need of psychiatric services acquire the skills necessary to liveindependently in the community. S/he works within the Agency\'s teammodel, establishing open communication, support, and accountabilityamong co-workers. REQUIRMENTS: Masters prepared unlicensed and/or veryexperienced Direct Care working with the adults experiencing mentalhealth symptoms (min. 2-3 years\' experience) Supervisory experiencepreferred ESSENTIAL JOB FUNCTIONS: Provides direct support andsupervision to Direct Care I and II staff Provides crisis interventionAdheres to program policy and procedure manual Participates in teamapproach, sharing responsibility for participants services Notifiessenior supervisory staff immediately in the event of a crisis, suspectedillegal activity, or any behavior constituting an incident in theprogram Assists participants with activities of daily living e.g.personal hygiene, meal preparation, housekeeping, laundry, moneymanagement, etc. Transports participants in matters related to servicedelivery, e.g. medical appointments, shopping, job interviews,vocational program, banking, laundry Administer medication accordancewith MAP policies and procedures Deals with entry and termination issuesTake advantage of a competitive salary of \$48,000 annually along with aphenomenal benefits package that includes Dental, Health and Lifeinsurance.
