Job Overview Cascadia Health seeks an experienced highly self-directed individual with excellent verbal and written communication skills to provide case management services for the twenty-six Cascadia Health residents at the Joyce. The Counselor III will participate in the long range planning and development, as well as day-to-day operations of the programs treatment services. The Joyces case manager is a highly compassionate and driven individual who will play a part in ensuring that services are delivered through a client centered and housing first model, specifically through the coordination of services and partnerships with local agencies, participating residents will receive help in obtaining needed supportive services such as mental health services, child care, homeownership counseling, credit clean-up, educational or vocational counseling, educational, assistance to maintain benefits through Oregon Health Authority(OHA), Supplemental Security Income (SSI) and Social Security (SS). The Joyce is a 66-unit permanent supportive housing (PSH) program located in the heart of downtown Portland, and is a program operating in partnership between Cascadia Health, Native American Rehabilitation Association of the Northwest, Cascade AIDS Project, and Community Partners for Affordable Housing. The Counselor III provides case management and clinical services to 26 of the residents at the Joyce, which is a PSH program serving individuals through the coordinated access system who have experienced homelessness, mental health challenges, including those with severe and persistent mental illness (SPMI), substance use disorders, and other challenges as a result of physical and emotional traumas. The Counselor III will work as a member of an interdisciplinary team to implement and monitor treatment for 26 Cascadia Health residents within the PSH program. This position involves participation and implementation of program goals and objectives to promote a strengths-based, client-centered approach to assisting residents in achieving individuals goals and ultimately help them to reach a place where they are able to make independent decisions and live autonomously. Essential Duties This position description is not intended to be an all-inclusive list of responsibilities, skills, or working conditions associated with the position. Provide therapeutic interventions and counseling with residents in order to support the residents with daily functions and symptom management. Coordinate with oncoming shift regarding resident concerns, building/program needs, and any issues relevant to ensure the smooth operation of the program. Provide individual and group treatment services as directed by the residents Treatment Plan; identify and prioritize with the client their treatment plan goals. Redefine goals as needed to obtain success; provide continuous assessment and measurement of progress toward treatment goals. Consult and collaborate with program leadership, and others as necessary, to maintain treatment team communication. Attend clinical rounds and case conferences to discuss progress of residents as well as track their treatment needs. Coordinate the treatment and continuity of care with external stakeholders. E.g. Guardians, medical team, and community based agencies Provide clinical consultation and guidance to other members of the interdisciplinary treatment team; meet regularly to discuss and address resident needs and concerns. Read and review residential assessments and treatment plans for final approval up to twice a year per resident to ensure continuity of care. Complete annual level of Service Inventory (LSI) as per the OARs Provide discharge planning with residents. Assess resident needs, provide housing resources, coordinate screenings, accompany residents to appointments, or new placements. Participate in the on-call rotation for directors custody evaluation; must have the ability to perform a directors hold and maintain required certification to do so Assist in crisis situations as appropriate, following emergency protocols and procedures. Provide verbal crisis de-escalation as appropriate, to avoid crises or to neutralize crises in progress. Compliance Adhere to mandatory abuse reporting laws and HIPAA requirements. Demonstrate responsibility for safety of clients, staff, and property; follow all established safety protocols and procedures; possess working knowledge of fire regulations and evacuation procedures. Complete annual employee training requirements on a timely basis as indicated in the licensing requirements for the facility as well as agency requirements. Respect and honor client rights and responsibilities and demonstrate professional boundaries and ethics. Document and maintain clinical records in a manner that assures compliance with all agency policies and procedures and local, state, and federal regulations. Complete mental health assessments and generate a treatment plan for each resident on an annual basis to support residents with their goals and transition to a lower plan of care. Qualifications Education: Must meet the requirements to be credentialed as a Qualified Mental Health Professional (QMHP) through MHACBO. Must have a Masters Degree in a behavioral science field from an accredited college or university. Demonstrate the competencies to become credentialed as a QMHP as outlined in the Oregon Administrative Rules (OAR). Certification must be current and in compliance with the OARs. Experience: One to three years of case management experience is preferred or any relevant experience work with homeless individuals who carry a mental health diagnosis, including those with Serious and Persistent Mental Illness (SPMI.) Specialized Knowledge, Skills and Abilities: Knowledge and understanding of regional homeless services. Knowledge of and appreciation for harm reduction principles Ability to provide de-escalation avoidance techniques. Advanced knowledge of culturally specific services Ability to perform a mental health assessment and service planning. Knowledge of Trauma Informed Care, evidence-based practices, and strength based philosophies. Knowledge of Microsoft office suite (I.e. Word, Power Point, Excel) with basic excel skills Must be able to operate computer programs and systems at a level needed to perform the essential duties. Knowledge of basic life saving measures and first aid Other: CPR Certification is required and to be maintained. Training is provided post hire to obtain. Working Conditions Environmental: Work is generally performed in a residential setting with occasional work in the community. Operating Hours: Work is generally performed during standard business hours, 8:30 am to 5:00 pm, in an office environment. Requires to be on call for one full week in a rotation with other on-call administrators. Per Diem (On Call), cover shifts as needed. Mental: Meal periods and breaks are subject to interruptions. Remaining aware and alert is critical to detect and address escalated and potential volatiles situation constantly. Universal precautions should be taken due to exposure to disease, biohazards, noise and contaminants Physical Demands: Work is regularly performed stationary at a computer; will be meeting with clients or doing documentation. There will be moving from milieu to an office setting, typing and utilizing a computer to document progress and type treatment plans and care. Regular walking up and down stairs as many sites have stairs. Must be able to position or stoop if needed to provide avoidance techniques as required. There is constant verbal communication in client interaction and with internal and external stakeholders. Must constantly be aware in this environment and be able to judge what is going on around you.
