Case Manager Permanent Supportive Housing Overview Title: Case Manager Department: Permanent Supportive Housing Responsible To: Program Supervisor Location: Alder House Apartments Status: Part Time and Full-Time Salary: $26 - $28/hr Purpose The Case Manager will work on a dedicated team with a direct caseload of participants in the Permanent Supportive Housing program. Permanent Supportive Housing (PSH)subsidized housing is where residents have a lease and access to voluntary, on-site case management, peer support services, and community activities through the Multnomah County Coordinated Access system, `managed by the Joint Office of Homeless Service (JOHS). To be eligible for PSH, one typically needs to be experiencing chronic homelessness, low income, and have one or more disabilities. Maintain consistent and reliable attendance to ensure continuity of services and support for clients and team members. Assist with resident screening, intakes, move-ins. Establish trust and develop a professional relationship with residents. Establish and maintain professional boundaries to effectively provide servicing to participants. Set routine, standing appointments for each resident on a weekly basis. Identify resident’s needs, establish goals and objectives, and create a mutually agreed upon individual support plan (ISP) as it relates to continual provision of housing stability. Connect participants with support and services as they relate to needs and ISPs. Ensure effective delivery of case management services by following established ISP, and keeping updated, concise case notes and proper documentation. Ensure documentation accurately reflects progression to maximize positive outcomes. Promote and encourage healthy ADL skill building activities through individual and group-based settings. Provide a resident centered approach to the provision of servicing and support which promotes independence and autonomy. Assist participants in applying for benefits. Care coordination for hospitalizations or other subacute level of care provision including support for admission and/or discharge planning and follow-up services. Respond to participant crises and provide de-escalation, coaching and education as needed. Provide education and professional referrals for services to increase overall quality of life, including referrals and promotion for physical and mental health. Collaborate with building Resident Service Coordinator and Community Health Navigators for continuation of support as appropriate. Positively promote and encourage a supportive culture reflective of agency values. Provide impromptu coaching, training, and support for program participants. Communicate change of conditions and areas of concerns for residents to Program Supervisor team member Coordinate with agency departments for collaborative servicing efforts such as vaccine clinics, health navigation, food security and emergency preparedness to provide maximum internal support for participants. Address volunteer opportunities with Program Supervisor Participate in routine meetings. Establish partnerships with community-based organizations and other identified partners to increase programming supports, enhance quality and quantity of servicing, and opportunities for team trainings and networking opportunities. Develop and maintain relationships with on-site partners. Seek understanding of contract and funding sources to aide Program Manager in program compliance and requirements Ensure spending accurately reflects ISPs and resident needs. Assist Community Health Navigators to understand resident needs. Oversee role specific duties to ensure and maintain HIPAA compliance. Assist in providing content to Program Supervisor for monthly newsletters Participate in the transportation and set up of goods for food security and on-site food pantries. Assist the Program Manager in data tracking. Participate in community events, meals and activities as assigned. Proven ability to engage with communities of color and the LGBTQ+ communities; Creates a welcoming environment and engages with people of varying backgrounds in group and individual discussions; Encourages and sparks conversation of equal/mutual participation. Skills and Abilities Sufficient manual dexterity and physical ability to perform assigned tasks Skill in case management, interviewing, assessing, planning, coordinating resources and implementing case plans. Familiar with case management best practices Experience making and monitoring quality referrals and collaborating across agency lines to guide clients towards setting and reaching their own goals within the context of available resources and creative problem solving. Demonstrated ability to work effectively, independently, and as part of a cross-disciplinary team. Ability to research community resources and mobilize/leverage resources to encourage client directed success. Physical Demands / Work Environment: Computer literate with working knowledge of Google Office. Must be able to sit, stand, walk, drive, talk on the telephone and/or use a PC for long periods of time. Occasionally required to stand and/or walk for extended periods of time; sit for extended periods of time; lift, push, pull and/or carry objects up to 50 pounds. Requirements At least one (1) year experience providing case management or direct services related to housing stability with diverse and vulnerable populations including homelessness, addiction, and mental health disorders. Experience with crisis management and navigation. BA degree in Social Work, Mental Health, Public Health or other related fields encouraged and not required. QMHA, CDAC, QMHP and/or other credentials are encouraged and not required. Any combination of education, work experience and lived experience will be considered. Ability to speak more than one language is preferred and not required. Must be over the age of eighteen (18). Must posses a valid driver’s license, current proof of auto insurance, and a reliable mode of personal transportation . Must be able to be lawfully employed in the US.
