Geriatric Occupational Therapist

Fast, Growing, Certified Home Care Agency (CHHA) is seeking a full time Occupational Therapist to provide home visits in Westchester to the geriatric population! Salary $110k - $115k + excellent health benefits, PTO and 401k RESPONSIBLITIES: Evaluates patients referred for home care services to determine appropriateness for Occupational Therapy services. Establish patient specific plan of care. Completes documentation accurately, and submits to agency office in a timely manner. Ensures patients safety and satisfaction. Hours: Monday - Friday 9am - 5pm QUALIFICATIONS Must be NYS Licensed Home Care experience preferred Experience with electronic medical record preferred, but not required.
