This role involves developing appropriate plans of care that adhere to all regulatory and clinical practice standards.\n\nKey Responsibilities\n\nUtilize standardized assessments and evidence-based practices to support clinical interventions.\nAssess patient progress and adjust treatments in accordance with regulatory and clinical requirements.\nEvaluate treatment outcomes for effectiveness, modifying services based on patient evaluations and referring as necessary.\nProvide consultation and counseling to patients, families, caregivers, and other service providers concerning speech disorders.\nCommunicate effectively with supervisors and team members regarding patient status, challenges, and treatment plans.\nPrepare comprehensive discharge summaries in line with regulatory requirements.\nDocument all clinically and regulatory required information in the electronic documentation system and maintain patient records according to company policy.\nAccurately record treatment minutes for services rendered per all relevant guidelines.\nParticipate in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings.\nEngage in mandatory trainings, in-services, and conferences.\nCoordinate the acquisition of necessary Durable Medical Equipment (DME) and supplies as per regulatory requirements.\nSupervise and support Clinical Fellows (CFY) or Student Speech-Language Pathologists.\nAdhere to the American Speech-Language-Hearing Association (ASHA) standards and state practice acts, as well as the code of conduct and ethics throughout professional practice.\n\n\nQualifications\n\n1 year of Speech-Language Pathology experience is required.\nExperience in Long Term Care (LTC) or Rehabilitation settings is required.\nA valid Massachusetts State License is required.\nMust hold certification from the American Speech-Language-Hearing Association (CCC).\nNew graduates are welcome to apply as long as they have Level II experience, and not just Level I observation experience.\n\n\nLocation\nThis position is located in Massachusetts.Modern Healthcare Solutions Job ID #36797023.
