PACE Home Care Coordinator (HHA)Direct Hire Opportunity
Monday – Friday | 8:00 AM – 5:00 PMKey Responsibilities• Coordinate meal delivery, caregiver services, and durable medical equipment (DME)
• Conduct home safety evaluations and home visits for participants
• Collaborate with nurses and the interdisciplinary care team
• Monitor and document services in the Electronic Medical Record (EMR)
• Advocate for participant needs and ensure quality care deliveryExperience Required
Minimum 1 year experience working with frail or elderly populations
Valid California Driver’s LicenseEducation & Certifications
CNA Certification
California Department of Public Health Home Health Aide (HHA) Certificate
CPR & First Aid Certification (American Heart Association)
