Virtual Diabetes Health Coach, Full-Time Stability Health™ is a company dedicated to providing a differentiated virtual diabetes care management model that enhances the primary diabetes provider’s ability to deliver better care with a lower total cost and helps patients to better self-manage their disease for improved quality of life. We are hiring health coaches with experience in diabetes for full‑time positions. The primary role will be in assisting with onboarding and assessing participants to customize their personal Stability Health program. You will have a panel of participants that you work with throughout their journey, providing communication back to their providers. Our ideal candidate is: A healthcare professional with 1‑3 years of diabetes health coaching experience An empathetic, active listener Knowledgeable with and has experience in behavioral change strategies Able to create a HIPAA compliant, home workspace Possesses excellent written and verbal communication skills Excited about working in a virtual environment that is evolving and growing A lifelong learner Detail oriented Independent and self‑directed Experience with and enjoy working with a culturally diverse patient and staff population. Able to coach to ADA Diabetes Standards of Care Job Description Outreach calls to potential participants Assisting with diabetes device onboarding and gathering data Engaging participants in collaborative relationships, empowering them to manage their psycho‑social, physical, and environmental health to improve and maintain their well‑being Review blood sugar data with participants and provide education and support within the participant’s care plan Working with participants to mitigate the impacts of social factors on health and their functional status Maximizing access to community diabetes and other resources available to the participant Providing education and coaching to participants as identified in their personal care plan to empower them to self‑manage their diabetes and improve their overall health Qualifications: Certification in Health Coaching from NBHWC or NSHC Minimum 2‑3 years of health coaching experience or 1‑3 year(s) of experience in outpatient diabetes care management Bilingual in English/Spanish with ability in both written and verbal communication Proficiency with technology/computers in web‑based applications Ability to work effectively in a virtual team environment Exceptional interpersonal, organization, and communication skills Preferred Healthcare professional with outpatient clinic experience: RD, RN, LPN, MA Completed or is eligible to complete the ADCES Diabetes Educator Level 1 or Level 2 through ADCES Competitive salary and benefits package (Medical, Dental, 401k etc.) Home Office Setup PTO and Holidays Teaming with an incredible company helping to close the gap in diabetes care Salary Range: ($53,000 - $64,000) Remote company - Timezone for work: MST (does not recognize Daylight Savings) Seniority level Entry level Employment type Full‑time Job function Health Care Provider Industries Hospitals and Health Care #J-18808-Ljbffr
