The role involves developing suitable plans of care in compliance with regulatory and clinical practice standards. Key responsibilities include: Utilizing standardized assessments and evidence-based practices to support clinical interventions. Providing rehabilitative, skilled, and medically necessary treatment interventions for patients with cognitive and/or ADL physical functioning disorders, adhering to all regulatory and clinical practice requirements. Administering various rehabilitation procedures, including but not limited to, ADL retraining, therapeutic activities, therapeutic exercises, and the use of supportive and assistive devices. Assessing patient progress and adjusting treatment plans accordingly, while maintaining comprehensive and accurate clinical records. Evaluating treatment outcomes for effectiveness and modifying services as needed based on patient evaluations. Offering consultations and counseling to patients, families, caregivers, and other service providers regarding physical disorders. Communicating effectively with supervisors and other healthcare team members about patient progress, barriers, and treatment plans. Providing a detailed discharge summary of services rendered, meeting all regulatory and clinical requirements. Documenting all necessary information in the electronic documentation system and maintaining patient records according to company policy. Recording treatment minutes accurately in compliance with all regulatory and clinical requirements. Participating in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings. Engaging in required facility and company trainings, in-services, and conferences. Obtaining necessary durable medical equipment (DME) or supplies as per regulatory and clinical practice requirements. Supervising and supporting occupational therapist assistants, rehabilitation aides, and/or students as needed. Adhering to NBCOT and state practice acts, as well as the code of conduct and ethical standards during professional practice as an Occupational Therapist.Qualifications Minimum of 1 year of occupational therapy experience. Experience in long-term care or rehabilitation settings is required. Must possess an active occupational therapist license.
