Children's Behavioral Health Home Coordinator / Targeted Case ManagerThe Children's Behavioral Health Home (BHH) Coordinator / Targeted Case Manager is responsible for overseeing and coordinating comprehensive care services for children and families with behavioral health needs.This full-time role blends direct client support with care coordination, documentation, advocacy, and collaboration with multidisciplinary teams to ensure high-quality, person-centered care.The ideal candidate is compassionate, organized, and skilled at engaging children and families — including those who may be hard to reach or hard to engage — while navigating complex systems of care.Essential Duties & ResponsibilitiesClient Assessment & Treatment PlanningEstablish and maintain therapeutic relationships with clients and families consistent with program philosophy and valuesDevelop individualized treatment plans in collaboration with families, guardians, and service providersMonitor and update treatment plans to reflect changing needsPrepare comprehensive treatment plans, quarterly updates, discharge plans, and required documentationSupport families in identifying goals and coordinating services for their childrenCase ManagementProvide integrated case management by assessing strengths and needs, coordinating services, and monitoring progressLead treatment teams and facilitate quarterly treatment team meetings (or as clinically indicated)Participate in safety planning and crisis intervention; provide enhanced access during evenings and weekends as neededEnsure compliance with all regulatory, program, and documentation standardsCoordination of ServicesCollaborate with community partners, including mental health providers, hospitals, residential facilities, schools, and social service agenciesEducate families about behavioral health resources and community supportsEnsure timely access to services for all referralsAdvocate for children and families within education, healthcare, and social service systemsTeam CollaborationAttend weekly BHH team meetings and monthly staff meetingsParticipate in clinical reviews and professional development opportunitiesMaintain productivity expectations and assist with program development initiativesGeneral ExpectationsMaintain confidentiality of client and agency informationFoster a welcoming, inclusive environment for individuals seeking servicesWork effectively with individuals from diverse backgrounds and abilitiesDemonstrate flexibility to meet client and organizational needs, including occasional overtimeWhy Join Our Team?Mission-driven work supporting children and familiesCollaborative and supportive team environmentOpportunities for professional growth and developmentCommitment to flexibility while maintaining quality careRequirementsThe position requires a bachelor's degree with a specialization in psychology, mental health, human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or a closely related field.OR have a bachelor's degree from an accredited four-year educational institution in an unrelated field and at least one (1) year of full-time equivalent relevant human services experience.Must complete the Child and Adolescent Needs and Strengths (CANS) assessment tool and pass the exam annually.Must possess a valid driver's license and reliable transportation.AMHC is an Equal Opportunity Employer.IND123
