Edenbrook Wisconsin Rapids promotes and encourages the success of each of its employees and values the individual experience of its guests and their families.Position: Hospital/Community LiaisonFriendly Village Nursing and Rehab, a skilled nursing facility in the Eden Senior Care Network, is now hiring a Hospital/Community Liaison responsible for overseeing outreach and referral development for Friendly Village and Edenbrook Wisconsin Rapids, serving both the Wisconsin Rapids and Rhinelander communities. This role focuses on building strong relationships with hospitals and community partners, facilitating patient referrals, and supporting census growth through strategic outreach and admission coordination.Key ResponsibilitiesHospital Liaison Responsibilities:Complete daily rounds in designated hospitalsObtain new high-acuity inquiries and assess patients to evaluate care needs prior to admission; direct referrals to the appropriate centerEvaluate inquiries, make admission-related decisions, and communicate information to the case manager and admissions coordinator to facilitate smooth patient transitionsObtain accurate information from physicians, patients, and referral sources regarding expected discharge plans and communicate updates to the interdisciplinary teamDevelop and maintain relationships with hospital social workers, physicians, discharge planners, administrators, clergy, attorneys, civic/professional organizations, home health agencies, and families to understand service needs and promote available programsBusiness Development & Community OutreachDevelop and maintain strong relationships with community organizationsConduct routine in-person visits to assisted living communities, senior centers, senior high-rise apartments, and physician officesRepresent the facility at networking events and community programsProvide education on facility services, specialty programs, and outcomesMaintain a consistent presence in assigned territoriesCensus Growth & StrategyMonitor daily census and proactively identify opportunities for growthAnalyze referral and discharge trends and adjust outreach strategies accordinglyParticipate in marketing planning and implementationQualificationsProven experience in healthcare admissions, marketing, business development, or case management (skilled nursing preferred)Strong understanding of Medicare, Medicaid, and managed care reimbursementDemonstrated ability to build and maintain relationships with physicians, hospital staff, social workers, discharge planners, and other referral sourcesSales and marketing experience with a track record of generating referrals and meeting performance goalsExcellent communication, negotiation, presentation, and relationship-building skillsKnowledge of local hospitals and healthcare networks preferred (willing to train)Valid driver’s license and reliable transportation for local travelCore CompetenciesResults-driven and competitive mindsetExceptional follow-up and responsivenessStrong organizational and multitasking skillsProfessional presence and presentationAbility to work independently and as part of a teamData-driven decision-makingCompassionate and resident-centered approachWorking ConditionsCombination of office-based work, hospital rounding, and community outreachLocal travel requiredOccasional evening or weekend community events may be required
