Nurse Practitioner (Per Diem)

Per Diem Rates:The telehealth visits for community = $150 (which is an average compensation).In person visits are compensated = $150- $210 (with $180 being an average FFS)Orientation hourly rate: $90/HourThis field position will cover Brooklyn and Queens regions.MJHS is a large not-for-profit health system in the Greater New York area. We also offer Elderplan/HomeFirst: health plans for Medicare and dual-eligible individuals. As a not-for-profit organization, many of our programs and services are made possible through the generosity of grateful families, corporate donors and grants, as well as our own employees.Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.In collaboration with physician, the NP will provide comprehensive assessment, including multifactorial complexphysical examination and psychosocial needs, self - management education and tools to better manage patient's multipleconditions. As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primarycare services to an assigned the at risk panel of Elderplan members who reside in the community. The NP will insuredevelopment of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives. The NP will act as clinical resource to the member, family and interdisciplinary team.Provide and/or coordinate primary clinical care services for assigned high riskmembers and facilitates supportive care during transitions from acute or sub-acute care settingActs as a team player, member's advocate, and a clinical lead while working closely withInterdisciplinary Team to coordinate member's care and optimize health outcomesActs as resource and contact for member/designated representative regarding all aspects of memberindividualized care plan and disease processMasters Degree in Nursing, graduate of accredited Nurse Practitioner program requiredOne to two years clinical experience in role of nurse practitioner requiredMinimum one year in geriatric specialtyPrevious clinical RN experience in acute, subacute and/or long term care setting preferredPrior experience in manage care a plusNYS NP License, Registration, DEA certificatesDriver license and car required
