Case Management Analyst

A company is looking for a Case Management Analyst (Remote). Key Responsibilities Conduct annual Health Risk Surveys to identify member needs for their Individual Plan of Care Inform care managers of newly identified health/safety risks or service needs Complete delegated care coordination activities within established timeframes Required Qualifications 2+ years in behavioral health, social services, or a related field Proficient in Microsoft Office Suite Access to a private, dedicated workspace High School Diploma with equivalent experience (REQUIRED) Associate's or Bachelor's Degree in behavioral health or human services (PREFERRED)
