Job Summary and ResponsibilitiesThis position will service Nebraska City for both Hospice and Home Health.As a Home Health RN, you will:Be a clinical leader. Apply your expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care.Craft individualized care. Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being.Collaborate and coordinate. Work seamlessly with a multidisciplinary team - physicians, therapists, and support staff - to ensure holistic care.Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.Innovate with technology.
