Family Practice - Without OB Physician

Primary Care Opportunity - Located in Fairfax, SC - Charleston 85m - Full-Time, permanent positionQualifications:MD or DO from a 4-year accredited medical school Board certified or board eligible (in process of certification) Active South Carolina medical license with prescriptive authority DEA certification with appropriate scheduled drug approvals BLS required; ACLS preferredKey Responsibilities:Conduct patient assessments and create comprehensive management plansOrder and interpret labs, imaging, and other diagnostic studies Document clinical encounters in the EHR, including after-visit summaries Participate in care coordination, referrals, and team-based workflow processes Educate patients on self-management and treatment adherence Address and document barriers to achieving health goals Participate in pre-visit huddles and ongoing clinical collaboration Provide emergency medical care and oversee medication dispensing when needed Supervise mid-level providers and consult with other clinicians as necessary Engage in community outreach including health fairs, immunization events, home visits, and sports physicals Maintain active membership in local, state, and national medical societies Attend staff/provider meetings, continuing education events, and case presentations Participate in quarterly chart reviews and Health Care Collaborative efforts Perform other duties as assigned by supervisorPractice Approach:Team-based care focused on patient preferences and outcomes Emphasis on care coordination and population health management Utilization of EHR for documentation and communicationby Jobble
