There will be a lunch hour between 12-1. Please ensure the bid reflects the below (NOTING THOUGH Friday's will be different so only 4 hours will apply on Friday). o Guaranteed hours: 8 per day (excluding Friday) o OT applies applies after 9 hours per day OT Structure for Coverage: Providers require OT hours to be preapproved. All OT should be preapproved by your clinic manager (Marci). Please seek preapproval via email with Marci. If you have any questions, please escalate to the PSJH admin team. Providers should be able to get the work completed in their scheduled time so excessive OT should not accrue. We could look at an option of a read/study rate if the providers are feeling like there is not enough time in their scheduled day to complete work but will visit this option only IF needed. Bone marrow biopsies preferred. REQUIREMENTS: _____________________________________________________________ Active CA license and CA DEA Board Certification BLS Certification PRIVILEGING: ___________________________________________________________________________ We are a outpatient clinic, attached to the main hospital and chemotherapy center. Privileges will be needed due to the clinic being attached to the main hospital. 2 years of clinical activity will need to be submitted with a focus on hematology oncology. The clinic activity has to attest to the privileges being requested. Please note that the privileging timeline will not begin until the RFA and application are turned back in. - Privileging Timeline: 90 days (average) - RFA due within 48-72 hours (request for application); - Hospital Privilege Application due within 5 business days - If temporary privilege is applicable, eligibility can only be confirmed after completed file (application and all verifications received) are sent to MSO - Deviation from this timeline may result in the provider being cancelled from the site. o All specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties.
