Travel RN - Home Health

American Traveler is seeking an experienced RN for a Home Health/Hospice position in rural Colorado, requiring a minimum of 3 years of experience and active CO or compact RN license.Job Details• Home health and hospice/palliative care setting in a rural Colorado community,• Caseload includes home health, hospice, and palliative care patients,• Day shift with call requirements of 1-2 nights per week and 1-2 weekends per month,• 13-week contract,Job Requirements• Active CO RN license or compact RN license currently in hand,• Current BLS/BCLS certification required,• Minimum 3 years of RN experience required,• Home health and/or hospice nursing experience required,• Valid driver's license and access to reliable personal transportation required (no public transportation available in the area),• Colorado Adult Protective Services (CAPS) background check required for consideration,Additional Information• Responsibilities include conducting initial and ongoing patient evaluations, developing and revising individualized Plans of Care, and documenting clinical and progress notes,• Coordinates patient services and communicates changes in condition to physicians and interdisciplinary team members,• Provides patient and family education and counseling related to nursing and care needs,• Supervises and teaches other nursing personnel as needed,
