American Traveler is hiring a float RN for a home health position in the Greater Boston area, requiring at least 1 year of certified home care and case management experience with an active MA RN license.Job Details• Home health setting covering the Greater Boston area, including Back Bay, Beacon Hill, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, and West End,• Day shift schedule, 8:00 AM - 4:30 PM,• Weekend rotation required once every 6 weeks,• Float position requiring travel to assigned patient cases,• EMR: Epic (charting),Job Requirements• Active MA RN license required - no pending or conditional licenses accepted,• All certifications must be AHA-issued,• Minimum 1 year of certified home health experience required,• Minimum 1 year of case management experience required,• Ability to travel independently to assigned patient cases,• Flu vaccine required; no religious or medical exemptions permitted,• 2 professional references required for consideration,Additional Information• Responsibilities include assessing patient conditions, developing and implementing care plans, and providing education and support to patients and families,• Performs admission processes and case management duties in accordance with MGB Home Care policies and standards of practice,• Requires thorough and timely electronic documentation and interdisciplinary communication,• Timekeeping system: UKG/Kronos,• No validated parking is available at Boston locations; public parking lots are available at the candidate's expense, and public transportation is recommended,
